Rhodiola rosea: a new entry in the dm+d dictionary
The NHS Business Services Authority (NHS BSA) has added Rhodiola rosea to the dm+d dictionary (version 7.1.0). Rhodiola rosea is a herbal adaptogen — an unlicensed botanical product — used traditionally to reduce fatigue and support stress resilience. It is not a licensed medicine in the UK and is typically available as a dietary supplement.
What is it used for?
Rhodiola rosea is marketed for:
- Fatigue and low mood (evidence limited)
- Stress-related symptoms (traditional use)
Common doses range from 300–600 mg orally daily, usually in divided doses. Standardised extracts typically contain 3% rosavins and 1% salidroside where specified. However, as an unlicensed product, quality and potency vary significantly between suppliers.
Event-medicine relevance
Rhodiola rosea is not appropriate for acute event-medicine use. Its onset of action is slow — typically 2–4 weeks of regular use — and it has no role in emergency management of fatigue, collapse, or acute mental-health crisis. However, responders may encounter users who self-administer Rhodiola without medical oversight. If a patient presents with acute symptoms, escalate to hospital care and document any herbal products they are taking.
Key cautions and interactions
Common side effects: headache, dizziness, dry mouth, and gastrointestinal upset.
Serious risks:
- Allergic reaction (rare)
- Serotonin syndrome if combined with serotonergic drugs (SSRIs, SNRIs, tramadol)
Important cautions:
- Pregnancy and breastfeeding (insufficient safety data)
- Bipolar disorder (may precipitate mania)
- Concurrent serotonergic medications
- Uncontrolled hypertension
- Autoimmune conditions
Drug interactions: SSRIs, SNRIs, tramadol, alcohol, stimulants (including caffeine), and monoamine oxidase inhibitors (MAOIs) all carry interaction risk. Serotonin syndrome is the most clinically significant concern.
