The MHRA’s ninth annual #MedSafetyWeek campaign runs from 4 to 10 November 2024. This year’s focus emphasises the critical role that Yellow Card reports play in preventing future harm and improving patient safety across the UK.

Why Yellow Card Reporting Matters

Every adverse drug reaction report submitted via the Yellow Card scheme contributes to a growing safety database. These reports help the MHRA identify patterns, trends, and previously unknown risks associated with medicines and medical devices. Your observations—no matter how minor they may seem—can be the piece of evidence that prevents serious harm to other patients.

Who Can Report?

Healthcare professionals, patients, and carers can all submit Yellow Card reports. Reporting is straightforward, confidential, and takes only a few minutes.

What to Report

  • Suspected adverse reactions to medicines
  • Defects or safety concerns with medical devices
  • Medication errors or near-misses
  • Quality issues with products

How to Report

Submit your report online at yellowcard.mhra.gov.uk or use the Yellow Card app. Paper forms are also available on request.
